Adoption Certificate

Required each time an SPDR dog is adopted.

Adoption Information

This is an optional online form that gives Breed Reps an easy way to provide information to the adopter about the dog, including its background if known, vaccination records, any medical issues or medications, current diet, and more. After you've added the information you have available, the form will be emailed to your inbox so you can either print it out to give to the adopter, or forward it to them by email.

Breed Info - Page Template

This MS Word document will walk you through the information needed to create an educational information page about your breed, which becomes available to the public via the "Learn More" links on our website’s “Available Breeds” page. Download the form, type in your info, and attach the completed document to an email, along with 3-5 photos, to Patty Carey: pattyc98053 at gmail dot com

Breed Rep / Co-Rep Application

Complete this form to be considered for representing a breed through SPDR.

Breed Rep - Add Additional Breed

Existing Breed Reps must complete this form to be considered for representing an additional breed through SPDR.

Breed Rep / Co-Rep Renewal

At the end of each year, existing Breed Reps and Co-Reps must complete this annual renewal form to continue representing their breed(s) through SPDR.

Dog Surrender Form

This form must be signed by an owner when transferring their dog over to SPDR for rehoming.

Dog Transfer Form

When a dog is being transferred from SPDR to another organization, this form must be completed and signed by both the SPDR Breed Rep and a representative from the organization that is receiving the dog.

Foster Home Application

This form is available online, or by downloading a paper / printable version.

Home Check Report Form

This form may be used for home checks to help a Breed Rep determine whether a prospective adopter’s environment is suitable for adoption.

Incident Report Form

This interactive form must be used to describe an incident involving a dog who is either currently under SPDR’s foster care.

Reimbursement Request Form

This form must be used each time a Rep wishes to obtain reimbursement for rescue-related expenses on a dog in SPDR’s care.

Special Needs Funds Application

Complete and submit this form to the Board to request approval on additional “special needs” funding for a specific dog.

Statistics Form - Individual

This interactive form (you can type directly into it, save and send by email) may be completed for each dog handled by an SPDR Breed Rep.

Statistics Form - Annual Summary

This interactive form (you can type directly into it, save and send by email) must be completed by an SPDR Breed Rep at the end of each year to provide a record of all dogs handled throughout that year.

Volunteer Application

This interactive form (you can type directly into it, save and send by email) is for anyone who wants to volunteer.
